Thyroid Radiofrequency Ablation (RFA)

A minimally invasive, non-surgical treatment for benign thyroid nodules and recurrent thyroid cancer — performed under ultrasound guidance without general anesthesia.

Radiofrequency ablation (RFA) is a cutting-edge, office-based procedure that uses heat generated by high-frequency electrical current to shrink benign thyroid nodules, relieve compressive symptoms, and treat locally recurrent thyroid cancer. Dr. Guttler has been at the forefront of bringing RFA to the United States, training with world-leading experts in Italy and Korea.

What Is Thyroid RFA?

RFA uses a thin electrode needle inserted into the thyroid nodule under real-time ultrasound guidance. Radiofrequency energy heats and destroys the nodule tissue from within, causing it to shrink over the following weeks and months. The surrounding thyroid tissue is preserved, and thyroid function typically remains normal.

Who Is a Candidate for RFA?

RFA is ideal for patients with benign thyroid nodules confirmed by fine-needle aspiration (FNA) biopsy that are causing symptoms such as difficulty swallowing, a sensation of pressure in the neck, or cosmetic concerns. It is also used for autonomously functioning thyroid nodules causing hyperthyroidism, and for locally recurrent thyroid cancer in patients who are not surgical candidates.

Advantages Over Surgery

Unlike thyroidectomy, RFA requires no general anesthesia, no incision, and no hospitalization. Patients return home the same day and resume normal activities within 24–48 hours. Thyroid hormone replacement is generally not needed because healthy thyroid tissue is preserved. The risk of damage to the recurrent laryngeal nerve or parathyroid glands — serious surgical complications — is essentially eliminated.

The RFA Procedure

The procedure is performed in the office under local anesthesia. Using the 'moving-shot' technique, the electrode is repositioned continuously within the nodule to achieve uniform ablation. Most sessions last 20–45 minutes. Mild discomfort, warmth, or a brief ache in the neck is normal. Patients are monitored briefly and then discharged.

Results and Follow-Up

Studies show that RFA reduces nodule volume by 50–90% over 6–12 months. Compressive symptoms typically improve within weeks. Follow-up ultrasound is performed at 1, 3, 6, and 12 months. A small percentage of nodules may require a second treatment session. Long-term data from Korea and Europe confirm durable results with an excellent safety profile.

Dr. Guttler's RFA Experience

Dr. Guttler trained with Dr. Roberto Valcavi in Italy and Dr. Jung Hwan Baek in Korea — two of the world's foremost RFA pioneers. He has been instrumental in introducing and advocating for RFA in the United States, where it remains underutilized despite its proven track record. He offers RFA consultations for patients seeking a non-surgical alternative.
