To All Endocrinologists,

A recent local endocrinologists referral, with persistent thyroglobulin and TG antibodies, had recurrent thyroid cancer found with our new techniques, in her neck, when WBS was negative as was MR, and PET/CT. I hope you can consider using me for your future “tough” diagnostic problems.

The recent advances in diagnosis and treatment of thyroid cancer have resulted in the need for a comprehensive endocrine neck ultrasound laboratory. Every thyroid cancer patient will need comprehensive lymph node mapping done before surgery, and for several years after surgery. Ultrasound and blood Thyroglobulin Cancer marker testing have replaced total body imaging in most thyroid cancer patients. Radiation therapy is being phased out in the majority of low risk cancer patients. The busy endocrinologist does not have the time to become expert in all the new techniques needed to care for their cancer patients. The ENUL will offer these services to endocrinologists.

The lab will only do the procedures requested by the endocrinologist, and will report directly to them the results of the study. There is no consultative function offered to the patient directly, unless the endocrinologist requests one. This is similar to a private freestanding MR/CT/PET center.

Richard Guttler, will remove his clinical thyroidologist hat, and only offer these services to the endocrinologist without consultation. He is the founder of Academy of Clinical Thyroidologists, ACT, early pioneer in the use of ultrasound for thyroid cancer follow-up, and therapy. He took part in writing the board questions for certification of endocrinologists, for American Association of Clinical Endocrinologists to begin in 2008. He has been certified, and recertified in all aspects of diagnostic and interventional endocrine neck ultrasound by AACE, and is a member of the American Institute of Ultrasound Medicine, AIUM.

The Endocrine Neck Ultrasound Laboratory will offer these procedures to endocrinologists. Doctor Guttler will perform all these procedures real time without an ultrasound technician.

  1. Real Time diagnostic ultrasound of the thyroid, parathyroid, and neck lymph nodes.
  2. Pre-operative Lymph node mapping in suspicious or positive FNA studies. This can change the extent of the initial surgery and prevent local recurrence years later.
  3. >5 mm abnormal nodes found during the mapping, can be referred back by the endocrinologist for a USG FNA and Lymph node washing for cancer markers.
    These include Thyroglobulin Tg, and Calcitonin. Positive nodes will add a modified neck
    To the original total thyroidectomy plan by the surgeon.
  4. A yearly follow up lymph node mapping done at EndocrineNeck.com, to go along with the physical exam, thyroid blood testing, and cancer marker done by the endocrinologist.
    5. USG Percutaneous Ethanol Injection, PEI, for recurrent cancer nodes, in patients that their endocrinologists have decided that no more surgery is indicated, or tolerated
  5. USG Methylene Blue Dye localization for the surgeon one hour prior to the surgery. Recurrence in a vital area that was operated on before, that could cause nerve or parathyroid damage. US Guidance places 0.1cc of dye, on the anterior surface of the abnormal node. The surgeon easily locates the node without wasting time hunting for it.
  6. USG PEI for benign thyroid cyst therapy. The need for surgery is replaced by this simple procedure. PEI also is used for recurrent non-functioning parathyroid cysts.

Experts are using less RAI/131 for low risk cancer patients, so you need to have an endocrine neck lab to help you manage your patients, and rely less on nuclear medicine’s total body imaging and radiation therapy.
